Fee and Payment Schedule:

• Our fees vary according to the procedures and laboratory tests that your individual appointment requires, and with the length of time your physician devotes to your care.
• We request that payment be made at the time of services. If circumstances prevent that, please speak to one of our staff and we will make every effort to arrange a satisfactory payment plan.
• If your account becomes past due, we will take action to recover the amount due within 90 days of initial billing.

Insurance:

As a patient you have full responsibility for payment of the services rendered regardless of insurance coverage. Our staff will be happy to help with all necessary insurance forms and answer any specific questions regarding fees and insurance.
