Appointments

• Our appointment desk is open from 9:00 a.m. until 5:00 p.m., Monday through Friday.



• Please be aware that our busiest phone times are early in the morning and late afternoon. It is never our intent to keep you waiting; we will answer as soon as possible.



• Please call well in advance when scheduling routine examinations and checkups. Calling in advance for a routine exam will give you a time that will meet your schedule best. When calling for an appointment, please indicate if there is a special problem, so we will be able to set aside sufficient time with your doctor.



• If it is necessary to cancel your appointment, please do so as far in advance as possible so we may give your time to someone else. We urge you to be on time so that we will not cause you or others undue waiting time. Remember, if you are late, it may delay everyone else.



• On entering our office, please go directly to the reception desk. If this is your first visit, you will be asked to complete and sign an information form. Please have all your insurance information, including a copy of your insurance card, with you when registering. If you are an established patient, please advise the receptionist when checking in on any changes in your address, phone number, marital status, insurance coverage, etc., that may have occurred since your last visit.
